Please add an item to your requests first.

Premium Surfaces accepts up to 10 sample requests at a time through our online request form. First, please browse our online catalog to find the product(s) of interest. You can use our "Catalog Search" features to the left, to make your search easier and faster.

Once you find a product you like, click on the thumbnail image of a product to view the product details. There you will see a "Sample Request" link. Click on the "Sample Request" link to add the product to your requests.

All products chosen for sample request will be saved while you browse our catalog. The products will be removed only after you submit the request for samples, or when you close your browser window. Complete the Request Form to send your request to us. You may order up to 10 samples per request form. Keep in mind that we do not take orders in real time online, but we do encourage you to ask for a quote at any time.
